Our Care Managers can assist you and your family in finding exactly what your loved one needs – whether medical, social or financial – to achieve and maintain the highest quality of life. Our Care Managers are professionals with degrees in social work, nursing and related fields, and have many years experience in geriatrics and long term care.
You may benefit from Care Management if:
- You are a long-distance caregiver to your loved one;
- You need assistance with coordinating care, services and appointments;
- You are concerned about the long-term costs of meeting your loved one’s needs;
- Your loved one is resistant to the changes you believe are necessary;
- You are trying to juggle the needs of your loved one with work and family;
- You are confused by the array of services and providers that are available;
- You are concerned that other professionals involved with your loved one’s care may not have the time to answer your many questions
Specific Care Management services may include:
- Comprehensive care assessment of your loved one’s current status and future needs
- Referrals for long term care and financial planning
- Assistance with eligibility for the Arizona Long Term Care System (ALTCS) and other long term care funding sources
- “Peace of Mind” visits – visits to your loved one by a Care Manager as frequently as you choose, with telephone and written feedback on the visit
- Accompany your loved one to medical appointments, and communicate results of visit directly with family
- Coordinate services and advocate for your loved one when you can’t be there
